This tool may capture portions of the conversation between myself and the dental team for the purpose of documenting my dental care. The information may be used to assist with clinical documentation, treatment planning, follow-up instructions, and healthcare operations.

I understand that the AI-assisted scribe or recording tool is used to support documentation and does not replace the professional judgment of the dentist or clinical team. My dental providers remain responsible for reviewing and finalizing my clinical record.

Communication by Text Message

I understand that Barry Road Dental Group may use text messaging to communicate with me about matters related to my dental care and account.
